I. RELEASE FORM

PURCHASE NECESSARY.

To be eligible to enter or win the contest a person must be enrolled the month of the promotion for a minimum of one week. However the prizes that are indicated are stated for a person who has signed up for a month. If a person wins the most amount of weight in a 2 weeks period then that person will only win up to the value of 2 weeks.

HOW THE CONTEST WORKS

Participants must be enrolled with Camp Technique for a minimum of one week and a maximum of four weeks. The person will participate in the Camp Technique program and will agree to weekly weigh ins if they want to remain in the competition.

A Note of Caution. Everyone, including the entrants to the Promotion, should seek medical advice before beginning the program. Participants in the Promotion may not enter if they have used Ephedra or any other weight loss supplements or medications unless prescribed by a physician to treat a pre-existing medical condition (medication and medical condition must be noted on the client profile. All participants may be required to sign an affidavit of eligibility confirming their compliance with the foregoing and these official rules.

Eligible entries will consist of all of the following components: (a) Filled Training Agreement, (b) Client Profile and (c) Release Form

By entering (or otherwise participating in this promotion), participants expressly agree and consent to the following:

Consult with a physician or health-care provider before starting the Camp Technique program.

Consent to the use of their names, video or other likenesses, testimonials essay and other biographical information (in whole or in part) for any and all purposes, including, but not limited to, advertising, trade and/or promotion on behalf of Camp Technique, without further limitation, restriction or compensation, unless prohibited by law. The submission of the entry also constitutes permission for Camp Technique to publish all aspects of the entry in any and all media worldwide without limitation or additional compensation to entrant.

CONSUMER DISCLOSURE

The Grand Prize is for 4 weeks free at our facility, up to the amount of the package that you originally purchased. (Maximum value of $16,450). Second prize winner will win 1 week at Camp Technique facility up to the amount of the package that was originally purchased (Maximum value of $4,850) Third prize winner receives a Polar Heart Rate Monitor (Maximum value of $300). No transfer or substitution of prizes except at Camp Technique's sole discretion. Prize values are in U.S. currency. All prizes will be awarded at the end of the month and will be valid for 2 years. Any portion of the prize not accepted by the winner will be forfeited. The prize may not be applied to a current package that the person originally signed up for. Prize may be redeemed based upon availability at Camp Technique.

ADDITIONAL CONTEST TERMS AND CONDITIONS

1. Promotion Participation. Participation in the contest is subject to enrolling in Camp Technique’s program during the month of the contest and complying with the terms of this Contest. Camp Technique reserves the right to refuse participation to any persons at any time in its sole and absolute discretion or terminate your participation in the Contest, immediately without notice to you and without liability to Camp Technique, for any reason.

2. Grant of License. By entering into this contest and giving your Authorized Content to Camp Technique, you are directing and authorizing Camp Technique to, and granting Camp Technique and its affiliates, subsidiaries, assigns, licensees and legal representatives, an irrevocable, perpetual, worldwide royalty-free, right and license to host, cache, route, transmit, store, tag, insert ad insertion software, copy, modify, distribute, perform, display, reformat, excerpt, analyze, and create algorithms based on the Authorized Content in order to (i) host the Authorized Content on Camp Technique’s servers, (ii) display, perform and distribute the Authorized Content, in whole or in part, all in any form, medium or technology now known or later developed, online or offline and by any means of delivery, for editorial, commercial, promotional and all other purposes in connection with Camp Technique products and services now existing or hereafter developed, including without limitation the right to adapt to streaming, downloading, broadcast, mobile, digital, thumbnail, scanning or other technologies and for syndication on third party sites; and in connection with each of the uses, if any, of the Authorized Content. This license gives Camp Technique the right to display your Authorized Content via streaming and/or downloading technologies.

3. Authority to Upload Content. By uploading or otherwise providing your Authorized Content to Camp Technique, you represent and warrant to Camp Technique the following: (a) you are at least 18 years of age (b) all of the information provided by you to Camp Technique to enroll and participate in the Contest is correct and current.

4. Your Obligation to Indemnify. You agree to indemnify, defend and hold Camp Technique and its respective directors, employees, and applicable third parties (e.g. relevant advertisers, syndication partners, licensors, licensees, consultants and contractors) (collectively "Indemnified Person(s)") harmless from and against any and all third party claims, liability, loss and expense (including reasonable legal fees, damage awards, and settlement amounts) brought against any Indemnified Person(s) arising out of, or related to or which may arise from your Authorized Content, your name and other provided information and Camp Technique’s authorized use of any of the foregoing. The Indemnified Persons may in their sole discretion control the defense, at your expense, of any claim indemnified herein. In the event that the Indemnified Parties determine not to control the defense of any claim hereunder, any Indemnified Person may join in defense with counsel of its choice at its own expense. You will not settle or resolve any such claim in a manner that imposes any liability or obligation on Camp Technique or affects Camp Technique’s rights in connection therewith without the advance written approval of Camp Technique.

By entering, you acknowledge you have read the Official Terms, Conditions and Rules governing the Promotion, and you agree to be bound by them.

I, ________________________________ hereby give Camp Technique permission to use my photograph and or testimony, in any available form of media, including internet and intranet, in part or in full, for advertising and promotional purporses.
